This document sets out how Life Skill Adventure and Support Services Pty Ltd (“LSAS”, “we”, “our” or “us”) handles personal information collected through our website, and the terms that apply when you use www.lifeskill.au.

LSAS is committed to protecting your privacy and handling personal information responsibly, lawfully and respectfully.
This Privacy Policy explains what information we may collect through our website, how we use and disclose it, how we protect it, and how you can access, correct or complain about our handling of that information.
By using our website or submitting an enquiry, you acknowledge that you have read this Privacy Policy.
We handle personal information in accordance with:
• the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs);
• the National Disability Insurance Scheme Act 2013 (Cth), including confidentiality obligations that apply to NDIS-related information;
• the NDIS Practice Standards, including Privacy and Dignity and Information Management;
• the NDIS Code of Conduct; and
• the Notifiable Data Breaches scheme.
This website Privacy Policy covers information collected through www.lifeskill.au and related online enquiry, referral and employment forms. Separate service agreements, consent forms and our internal Information Management Policy govern how we handle participant records once services commence.
2. Information we collect
When you submit an enquiry, referral, employment application or other form through our website, we may collect information including:
• your name;
• your email address and telephone number;
• your preferred method of contact;
• your relationship to a participant (for example, participant, family member, nominee, support coordinator or referrer);
• the type of support or service you are interested in;
• information about an NDIS participant, where relevant to your enquiry (which may include name, date of birth, NDIS number, plan dates, goals, funded supports and living or support arrangements);
• information about a participant’s support needs, availability, communication preferences or service preferences;
• employment-related information where you are enquiring about work with LSAS; and
• any other information you choose to include in your message or supporting documents.
Please only provide information that is reasonably necessary for us to respond to your enquiry. Where you provide personal information about another person, you should ensure you have their consent or are otherwise authorised to provide it (for example, as a parent, guardian, nominee or authorised representative).
When you visit our website, our website hosting provider may automatically collect limited technical information, including your IP address, browser type, device information, pages visited and basic server logs. This information may be used to maintain the security, performance and operation of our website.
3. Sensitive information
Some enquiries may include sensitive information as defined in the Privacy Act. This can include information about a person’s disability, health, diagnosis, medication, behaviour support needs, cultural background, racial or ethnic origin, or other support requirements.
We will only collect sensitive information where it is reasonably necessary for our services and where the person (or their authorised representative) has consented, or where collection is otherwise permitted or required by Australian law.
You do not need to provide detailed health, disability or support information through our general website enquiry form unless it is necessary for your enquiry. We prefer to collect detailed participant information through a consented service onboarding process.
4. How we use your information
We may collect and use personal information to:
• respond to enquiries;
• provide information about LSAS and our services;
• consider and respond to service referrals;
• assess whether LSAS may be able to meet a prospective participant’s support needs;
• prepare service information, quotes or proposals;
• arrange meetings, assessments and service commencement activities;
• administer bookings and service delivery;
• communicate with participants, nominees, family members, support coordinators and other authorised representatives;
• respond to employment, volunteering or contractor enquiries;
• maintain the security and functionality of our website;
• improve our services and communications;
• manage complaints, feedback, incidents or concerns; and
• meet our legal, regulatory, safeguarding and reporting obligations, including obligations to the NDIS Quality and Safeguards Commission.
We aim to collect only the personal information that is reasonably necessary for our functions and services.
We will not use personal information for an unrelated purpose unless we have obtained consent or the use is otherwise permitted or required by law.
We do not use personal information collected through our website for direct marketing unless you have requested information from us or we have obtained consent. You may opt out of further communications at any time by contacting us.
5. Disclosure of personal information
LSAS does not sell, rent or trade personal information.
Where reasonably necessary, we may disclose information to:
• authorised LSAS employees, contractors or representatives (on a need-to-know basis);
• website hosting, information technology, email and communications service providers;
• professional advisers, insurers, auditors or consultants;
• government, regulatory or law enforcement bodies where required or authorised by law;
• the NDIS Quality and Safeguards Commission, the National Disability Insurance Agency, or other relevant authorities where LSAS has a reporting, quality or safeguarding obligation; and
• other parties where you have provided consent or requested that we share the information.
Third-party providers are only given access to information necessary to perform their services and are expected to handle that information securely and appropriately.
We do not routinely disclose personal information overseas. If a service provider stores or processes information outside Australia, we will take reasonable steps to ensure the information is handled in a way that is consistent with the Australian Privacy Principles.
6. Anonymity and alternative contact
Where it is lawful and practicable, you may deal with us anonymously or using a pseudonym. In many cases this will not be practicable if we need to respond to an enquiry about supports, employment or a specific participant.
7. Cookies
Our website may use cookies and similar technologies to support basic website functionality, maintain security and improve the browsing experience.
Most web browsers automatically accept cookies. You can usually change your browser settings to decline or delete cookies. Disabling cookies may affect the operation of some parts of the website.
8. Information security
LSAS takes reasonable steps to protect the personal information we hold from misuse, interference, loss, unauthorised access, modification and disclosure.

• limiting access to authorised personnel on a need-to-know basis;
• using password-protected systems and appropriate access controls;
• using reputable website hosting, email and information technology providers;

• staff training on privacy, confidentiality and the NDIS Code of Conduct; and
• securely disposing of information when it is no longer required, subject to applicable record-retention requirements.
While we take reasonable precautions, no method of transmitting information over the internet or storing information electronically can be guaranteed to be completely secure. We therefore cannot guarantee the absolute security of information submitted through our website.
If you believe personal information has been accessed, used or disclosed without authorisation, please contact LSAS promptly.
9. Data breaches
If we become aware of a data breach that is likely to result in serious harm, we will assess the incident and, where required under the Notifiable Data Breaches scheme, notify affected individuals and the Office of the Australian Information Commissioner as soon as practicable. We may also notify the NDIS Quality and Safeguards Commission where the incident is reportable or otherwise required.
10. Retention of information
We keep personal information only for as long as it is needed for the purpose for which it was collected, or as required by law and NDIS record-keeping obligations. Health and disability support records are generally retained for at least seven years (and longer where a participant is a child or other legal requirements apply). Website enquiry records that do not proceed to service may be retained for a shorter period and then securely destroyed or de-identified.
11. Accessing or correcting your information
You may request access to the personal information LSAS holds about you or ask us to correct information that you believe is inaccurate, incomplete or out of date.
Requests can be made using the contact details below. We may need to verify your identity before providing access or making a correction. Where a request relates to a participant, we will deal with the participant or their authorised nominee, guardian or representative.
We will take reasonable steps to respond in accordance with applicable Australian privacy laws. In some circumstances, access may be limited or refused where permitted by law. If this occurs, we will generally explain the reason for the decision.
12. Making a privacy complaint
If you have a concern or complaint about how LSAS has handled your personal information, please contact us using the details below.
We will acknowledge and review your concern and aim to provide a response within a reasonable timeframe. We may contact you if further information is needed.

LSAS may update this Privacy Policy from time to time to reflect changes to our services, systems, practices or legal obligations.
The current version will be published on our website and will take effect from the date of publication.
Part B — Website Terms and Conditions
1. Agreement to these terms
These Website Terms and Conditions (“Terms”) apply to your use of www.lifeskill.au and any enquiry, referral or employment form on that website.
By accessing or using the website, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the website.
2. About LSAS
Life Skill Adventure and Support Services Pty Ltd (ABN 41 664 738 201) is a registered NDIS provider. Our website provides general information about our services and allows people to contact us about supports, referrals or employment.
Information on the website is general in nature. It is not personal advice, an NDIS plan review, a service agreement, or a guarantee that we can accept a particular referral.
3. Website enquiries are not a service agreement
Submitting an enquiry or referral does not create a contract for NDIS supports and does not oblige LSAS to provide services.
If we are able to assist, any supports will be provided under a separate written service agreement (and any required consent forms) consistent with NDIS rules, including applicable NDIS Pricing Arrangements and Price Limits.
4. Accuracy of information you provide
You must ensure that information you submit through the website is accurate, current and provided with lawful authority. You must not submit another person’s personal or sensitive information unless you are authorised to do so.
We may decline, pause or close an enquiry if information is incomplete, misleading, or cannot be verified.
5. Acceptable use
You must not:
• use the website in a way that is unlawful, harmful, harassing or misleading;
• attempt to gain unauthorised access to the website, our systems or other users’ information;
